•Social Security Card or Year end Social Security Benefit Statement
•Tax Documents (1099s, W-2, SSA-1099, Broker Statement)
•Last Year Tax Return
•Voided Check
Registration is required for statistical purposes, but service is provided on a first-come, first-serve basis.
